Job Description:

This position provides intake and referral services to a comprehensive Behavioral Health team within a safety net healthcare setting. The Coordinator will conduct intake assessments to determine the need for a range of Behavioral Health services or referral to outside agencies. Additionally, this person will provide case management services to individuals enrolled in behavior health services on an as needed basis. This position adheres to the National Association of Social Workers (NASW) professional code of ethics and practice standards, or the APA Practice Standards and Code of Ethics, as applicable.

Essential Functions:

  • Conducts on-site and over the phone eligibility screenings and assessments for patients referred to the Behavioral Health program.
  • Facilitates referrals to internal Behavioral Health providers for services related to mental health needs; schedules clients for initial psychiatric appointments;
  • Applies sound judgment in reviewing medical necessity for continuity of care, and most appropriate referral to treatment;
  • Makes appropriate referrals to outside agencies when client needs dictate alternative service providers.
  • Works collaboratively with Clinic staff and local human service organizations, treatment centers, and other community based organizations in order to facilitate appropriate care coordination, information, and referral.
  • Provides Case Management services to small client caseload when the need is identified at intake/screening or during the course of treatment;
  • Accurately and in a timely manner documents appropriate health records in the electronic medical record;
  • When qualified, assists in the supervision of the Clinic's behavioral health volunteers and/or students as needed.
  • Participates in the review of clinical issues and program policies and procedures.
  • Supports quality management and program evaluation activities through ongoing quality improvement process engagement.
  • Supports the mission and vision of the Kansas City CARE Clinic, is a team member of the Kansas City CARE Clinic; supporting other staff members when needed.
  • All duties as assigned by Behavioral Health Director or Senior Staff Members when required for the smooth operation of the Clinic and/or its Administrative functions. Perform other duties as assigned that are related to behavioral health programming.

Physical Demands/Working Conditions:

  • This position may be exposed to virus, disease and infection from patients in working environment.
  • While performing the duties of this job, the employee is regularly required to identify, communicate and exchange information.
  • The employee regularly is required to move about the building.
  • May be expected to travel to community meetings, client homes or other agencies.
  • May experience traumatic situations including psychiatric traumatized and deceased patients.
  • Employee may regularly have to lift up to ten pounds.

Position Type: Full-time

Experience and Skills:

  • Bachelor’s degree in Social Work, Psychology, related field, or 2 years’ experience in behavior health / social service provider role;
  • Must have excellent interpersonal skills with ability to relate to others from diverse backgrounds;
  • Knowledge of mental health conditions and services designed to treat these conditions preferred;
  • Minimum 6 months Case Management experience preferred (this may include a school based practicum);
  • Knowledge of working with clients who have chronic health conditions, including HIV/AIDS a plus.
